Abstract

Previous research shows that unhealthy diets, particularly the Western diet, (Keum and Giovannucci, 2019, PubMed ID 31455888) are associated with an increased risks of colorectal cancers. This project seeks to develop a simple method to help individuals at increased risk of colorectal cancer improve their diets. The trial evaluates three different eating plans over 12 months: Full, Simple and Self-guided. The goal is to identify which eating plan results in the optimal participant adherence with co-primary endpoints of weight loss and the Preventative Eating Score. Secondary aims of this project are to evaluate if skin carotenoid and breath ketone measurements can be utilized as non-invasive measures of diet. As part of study setup, dietary intervention materials were designed. This included user manuals and handouts, which had to be prepared with a diverse population in mind. In addition, standard operating procedures had to be written and validated for the instruments that will be used for assessing the biological endpoints of carotenoid concentrations, breath ketones, and body fat. The trial will recruit 240 overweight or obese participants at increased risk for colorectal cancer, as defined by a strong family history of colorectal cancer or a genetic predisposition to colorectal cancer. The hypothesis is that the Simple Eating Plan, with limits four categories of Western foods, will result in similar outcomes as the Full Eating Plan that both limits Western foods and encourages consumption of preventive foods. Recruitment will start in fall of 2022. The long term goal is to identify the most successful diet plan for future implementation in primary care settings.
