Abstract
Stroke is one of the leading causes of adult disability in the United States, and many survivors (up to 70%) go on to develop some degree of post-stroke cognitive impairment and dementia (PSCID). Though physicians are becoming more efficient at detecting cognitive impairment post-stroke, little is known about which specific US populations are more susceptible to developing such problems. The DISCOVERY (Determinants of Incident Stroke Cognitive Outcomes and Vascular Effects on Recovery) study is a prospective, multicenter, observational, nested-cohort study of 8000 nondemented ischemic and hemorrhagic stroke patients with the goal of investigating the mechanisms of brain resilience while understanding the contribution of prevalent preexisting brain pathology. Participants will be followed up to 48 months, and will experience cognitive evaluations and biomarker testing. The main goals of this study include identifying the independent and combined effects of the acute stroke lesion, pre-existing SVD (cerebral small vessel disease) and AD/ADRD (Alzheimer’s Disease and Alzheimer’s Disease Related Dementias) on PSCID, as well as determining whether acute stroke interacts with AD/ADRD and SVD pathology leading to PSCID. Ultimately, the study hopes to develop personalized-medicine models to predict individual patient cognitive outcomes and select patients for future clinical trials. Adult patients within 6 weeks of acute stroke are eligible for participation in DISCOVERY. The focus of my abstract will be the discussion of challenges and strategies associated with recruiting an acute stroke population.
