Abstract
Soluble urokinase plasminogen activator receptor, (suPAR) is the cleaved form of uPAR, a protein found in migration of immune cells, shown to be upregulated during immune activation. suPAR levels thus directly affect immune activity and make suPAR an ideal biomarker for risk and a potential target for the treatment of kidney and cardiovascular disease. High suPAR levels are correlated with increased inflammation, severe disease progression, and fatal outcomes in various patient populations, including patients with heart failure. SuPAR levels have also shown to predict outcomes in patients with heart failure. The SOLOIST trial relates the impact of sotagliflozin and other SGLT inhibitors in patients that have had or currently are presented with Type 2 Diabetes Post Worsening Heart Failure through plasma samples. Through unknown mechanisms, SGLT inhibitors have also shown to improve outcomes associated with high suPAR levels in a subset of patients, specifically those with heart failure and kidney disease. Through ELISA assays, we can measure the concentration of the suPAR biomarker in plasma samples. These data show that there was a significant decrease in suPAR levels in the sotagliflozin treatment group. We believe that SGLT inhibitors can reduce suPAR levels and that the reduction in suPAR from SGLT inhibitors is related to the improvement of cardiovascular and kidney-related outcomes in patients. By using ELISA assays to provide insight to whether SGLT inhibitors impact outcomes relating to suPAR levels, the mechanisms behind the biomarker pathways as well as the use of SGLT inhibitors are better understood.
