Abstract
Following anterior cruciate ligament (ACL) injury quadriceps weakness is a frequent occurrence. Additionally, following ACL injury there is a large effusion present in the knee, but it is unknown if this effusion is related to the decrease in quadriceps strength. The purpose of this study was to determine if the magnitude of effusion after an ACL injury will influence quadriceps muscle strength. Patients who had recently sustained a primary unilateral ACL tear were included in this study. Patients completed a knee ultrasound and a bilateral quadriceps strength test within 90 days of their injury. Ultrasound images were taken longitudinally along the proximal pole of the patella to view the supra-patellar pouch. Cross sectional area of the suprapatellar pouch space was then measured to determine knee effusion. Patients also underwent a maximal effort isokinetic quadriceps strength testing, and a between limb symmetry score was calculated. We used a linear regression analysis to determine if patients with a larger knee effusion were more likely to have greater quadriceps strength asymmetries. We anticipate our results show that those with a larger suprapatellar pouch cross sectional area are more likely to have greater quadriceps asymmetries. Effusion within the knee is related to greater quadricep weakness, hindering the rehabilitation process for those injured. Therefore, effusion influences strength and special attention will be needed to minimize effusion post-injury.
