Abstract
Introduction: Shared-Decision Making (SDM) is a key component of medical best practice. It occurs when patients and clinicians work together to decide on an optimal treatment. However, the widespread recommendation of SDM in medicine did not occur until recently. Our research focuses on looking at health care guidelines for the US general internist to determine how often healthcare recommendations take into account patient beliefs and goals. Methods: As a part of a larger systematic search, we conducted a guideline review using the ECRI Guideline Trust database (a large guideline repository). We screened all guidelines using the Internal Medicine filter in ECRI and excluded any that were published before 2010 or did not originate from the US. We then conducted a full text search of the remaining guidelines for “shared decision making” or patient “preferences” or “values.” We then recorded the title, organization, publication year and whether a recommendation for SDM was included. We then assessed the frequency of recommendations for SDM among the included guidelines. Results and Conclusions: 1128 guidelines were screened for eligibility. 464 were excluded, leaving 664 for full text review. 10 guidelines could not be accessed. Of those guidelines that underwent full text review (n=654), roughly half (51.5%) included recommendations for SDM. The quality and appropriateness of these recommendations and their ability to guide clinicians in promoting patient-centered care will be assessed in future research.
