Abstract
Diabetes affects millions of people, and its management is shaped by various social and economic factors. Many individuals struggle to meet blood sugar and blood pressure targets, with nearly 40% facing financial challenges related to their condition. Additionally, a significant portion cannot afford necessary medical expenses, contributing to disparities in healthcare access and outcomes. While addressing financial and social barriers is important, it does not fully resolve the challenges of diabetes self-management. Many individuals find it difficult to maintain healthy habits, adhere to medications, and consistently monitor their condition. Without proper support, these difficulties can lead to complications, unnecessary hospital visits, and rising healthcare costs. This study explores whether combining financial navigation tools with peer support can enhance diabetes management. TailorMed Software, previously used in cancer care, helps address financial challenges, while peer support has been shown to improve self-care behaviors. Using a two-group study design, this research will assess how effective and practical these approaches are over six months. By identifying obstacles and evaluating the impact of these interventions, this study seeks to improve diabetes management and reduce health disparities.
