Abstract
STATEMENT OF PURPOSE: Research suggests sport-related concussion (SRC) risk and mechanism of injury (MOI) differ by sex and sport; thus, we aim to describe SRC epidemiology in men’s and women’s soccer to enhance safe participation in the sport. METHODS & APPROACH: The Ivy League-Big Ten Epidemiology of Concussion Study, a prospective cohort study, initiated in 2013/14. Over 400 Men and Women players participated, with most common positions being midfield, defense, and attack, and a less common position was goaltender. We described SRC incidence per 100-athletes with 95% confidence intervals. We classified MOI via percentages: activity type, head impact object, and coded descriptive narratives to contextualize MOI. We summarized median days (interquartile range:(IQR)) between injury to clinical recovery milestones (i.e., injury reporting, and return-to-academics, -symptom resolution, -limited sport, -full sport(RTP)). RESULTS & CONCLUSIONS: Average SRC incidence for Men (n=209) was 6.0/100 athletes (range:2.8-10.1), and Women (n=137) was 4.6/100 athletes (range:3.4-9.0). Men’s most common MOI was person-contact (56.94%, n=119), of which 30 occurred in competition 25.2%); women’s was ball-to-head contact (39.42%, n=54) of which 75 occurred in practice (54.7%). Men (70.81, n=148) and women (66.42%, n=91) often reported SRCs on the same day. Regarding recovery in Men, 43.5% n=91 had symptoms resolved in one week, and 23.4% n=49 resolved in 2 weeks. Regarding recovery in Women, 35.77% n=49 had symptoms resolved in one week, and 33.58% n=46 had symptoms resolved in two weeks. INNOVATION AND SIGNIFICANCE TO THE FIELD: We observed unique injury mechanisms, incidence, and clinical recovery between groups, warranting sex- and position-specific prevention strategies and policy considerations to optimize athlete safety.
